Uploading the Consent Form

How to upload a consent form:

  1. Open the protocol in IRBWise. You should have the protocol summary open on the screen. 
  2. On the right side of the screen you will see three buttons that say "edit," "print," and "submit." You will click on the button labeled "edit."
  3. The protocol will reload on the screen in an editable format.
  4. Scroll down to Roman Numeral III, titled “Subjects Information.”
  5. Scroll to question B, Proposed Consent Procedures.
  6. Click on the link to Specify Consent Procedures. 
  7. A new window will open. Scroll to the bottom of the window and click the link that states, “Upload/Mail/Fax Consent Forms."
  8. A new window will open where you can upload the consent form. (Only the most current consent form should be uploaded. Consent forms that are outdated because of updates or revisions can be deleted.)
  9. The application will need to be saved and submitted. The “History" section of the application can always be checked to be sure that the protocol is in the correct place.
