Submitting a Study Closure
How to close a study:

Log-in to IRBWise and open the study.

The protocol should be open on the screen. In the middle of the page, on the left, you should see the heading “Study Closures”. Click on the link that states, “Report Study Closure”.

The Study Closure form will open.

Because this form serves as a final report all of the questions need to be answered.

Then scroll to the bottom of the screen and click the button that states, ‘Continue >>>’.

That will open a summary of the closure application. Scroll towards the bottom and click the button that states, ‘Continue >>>’.

In the endorsements, check the box to agree to the statements and type in your name.

Select the option to submit the protocol directly to the IRB, then click the button that states, ‘Finish’.
