Sport Club Participant Agreement

In exchange for the opportunity to participate in GSU sport clubs and the benefits that shall accrue to me by virtue of this participation, I, *(type in full name), agree to abide by GSU’s terms and conditions as set forth below:

1. I agree to conduct myself in an appropriate and sportsman-like manner in all competitive and/or non-competitive sport club events that will reflect positively upon my Sport Club, Georgia State University, Recreational Services and the Sports Club Council.
2. I agree to comply with the Georgia State Student Code of Conduct. (Available online at http://www2.gsu.edu/~wwwcam/ and in the current edition of the On Campus Student Handbook)
3. I agree to abide by the rules of the Sports Club Council, the constitution of my sports club and any other rules that govern my club sport.
4. I will abide by all local, state, and federal laws.
5. I agree to refrain from all consumption of alcohol or illegal drugs at recognized sports club activities, including related travel, attendance, practice, and/or competition at sport club events.
6. I agree to refrain from consuming alcohol at public establishments while wearing club/team apparel.
7. I agree to refrain from engaging in any form of hazing. Hazing is defined as any intentional, negligent or reckless action, activity or situation that endangers or is likely to endanger the physical health of an individual or causes individual or causes an individual pain, embarrassment, ridicule or harassment as a condition or precondition of gaining acceptance, membership, office or other status in a student group, regardless of the individual’s willingness to participate.
8. I agree to refrain from participation in any activity that would deem harmful or destructive to any persons or private property. I understand that I am personally responsible for any damage to property including, but not limited to, vehicles, lodging, tournament sites, recreation centers and/or University/club sports equipment.
9. I understand that membership dues may be required to join certain clubs. My dues will be used to off-set the cost of equipment, uniforms, travel, supplies, and in some cases coaching. I fully understand that sport club membership dues paid to sport clubs are non-refundable.
10. I understand and agree that violation of this Agreement may result in the probation, suspension or removal of my club and/or its individual members from the Sport Club program.
