Honorarium Statement
To demonstrate our appreciation for your participation, each external reviewer receives an honorarium of $1,000. The external reviewer who serves as chair of the external review team receives an additional $200.
To facilitate processing of honorium payment, each external reviewer is asked to complete a W-9 form (Click here for the W-9 form). The W-9 form should be mailed to the following address:

Reviewers who are not US citizens will need to complete the Foreign National Information Form for tax purposes. Instructions may be found on the form.
