What is the effective date of my vision coverage?
Your vision coverage is effective the first of the month following your submission of the enrollment form.
Do I have another opportunity to enroll in vision coverage if I do not complete my benefits election form within the first 31 days of employment?
Yes, you can enroll in the United Healthcare Vision plan during the annual open enrollment period.
Is the vision plan an HMO?
Yes, the vision plan is an HMO. Participants can only use the providers listed on the United Healthcare website.
