Disability

Short Term Disability: This voluntary plan allows you to receive a benefit if you are ill or injured and unable to perform your job for at least 15 days, up through 13 weeks. The benefit covers 60% of your salary (up to a maximum weekly benefit of $600). You pay the full cost for this program. If you apply at any time other than at your time of hire, including during the annual Benefits Open Enrollment period, acceptance is contingent upon your providing evidence of insurability.

Long Term Disability: This voluntary plan allows you to receive a benefit if you are ill or injured for an extended period and unable to work. You may receive 60% of your salary (up to a monthly benefit of $7,500) after you have been out of work for 90 days. Benefit levels may be adjusted for other income such as retirement or Social Security disability benefits. You pay the full cost for this program. If you apply at any time other than at your time of hire, including during the annual Benefits Open Enrollment period, acceptance is contingent upon your providing evidence of insurability.

Instruction on Submitting a Disability Claim Online:

  1. Go to: http://www.prudential.com/gi
  2. Click the “Employees” tab on the right of the screen
  • Under “Service Center” click Employee Forms
  • Under “Self Service” click Disability Claim Submission
  • Click “Here” to submit disability claim
  1. Enter policy number “61605”, and State of Residence, then click “next”
  • Division Name/Work Location: enter the department you work in, and click “next”
  1. Complete the following:
  • Employee information
    • Client Employee ID: Enter Social Security Number
  • Other Income & Workers Comp
  • Job Information
  • Primary Care Physician
  • Medical Information
  • Disability Information
    • Branch number for STD & LTD is 00002
  •  After completing the online disability clain, click the "Submit" button.
  • Finally, click the following link: http://www.prudential.com/view/page/public/12165
    • Under “Disability and Absence Claims” select the “Attending Physician Statement” form.  Print the physician’s form, submit to your healthcare provider for completion and submit to the HR Benefits Office
