TCP/IP Host Registration

If you are a self-administered departmental LAN and wish to administer your TCP/IP network, you need only fill in your departmental name, network administer name, phone and subdomain name for the subnet registration. Individual forms must also be submitted for all workstations in your LAN. This IP Address is unique and should not be given out. Doing so may adversely affect the use of the network.

Note: The Internet Address must be a six digit number between 1-254.  The Network Adapter Address must be 12 digits; ranging from 00 - FF.
