How do I request an Online Web survey?
Please call the IS&T Help Center at (404) 413-HELP (4357) or e-mail help@gsu.edu to request an online survey. A technician will contact you shortly to begin the survey set-up. This process normally takes from 24 to 48 hours.
Please be prepared to provide the following required information:
1. A copy of the survey in electronic format (preferably MS Word) with all mandatory questions marked
2. If the survey will be distributed to Georgia State faculty, staff or students, your survey approval from the office of Institutional Research (Please see the new policy for survey approval.)
