ISSS Forms

Use the following documents only after you have checked to see if additional documentation is required. In order to view the following information, you will need Adobe Acrobat Reader. 

• Certification Letter Request -- To request certification letter from ISSS for NON-academic issues
• Change of Status Procedures -- Information on changing to F-1status in the United States
• Curricular Practical Training (CPT) Form -- Information on CPT employment authorization
• Document Shipping Instructions -- To request expedited mailing of documents (e.g. FedEx) or authorize third-party pickup
• Economic Hardship Information -- Information about off-campus employment authorization due to unforeseen financial hardship
• GRA Application -- Application for GRA position in ISSS.  Must be accompanied by a current resume.
• H-1B Request Packet -- For use by GSU departments who wish to hire a foreign national holding Bachelor's degree or above 
• ISSS Procedures -- Common services provided by ISSS to international students
• Invitation Letter -- Sample for international students to use to invite family to visit U.S. (ISSS does not write this letter.) 
• I-20 Request -- To request a new/updated I-20 form (e.g. financial update, change of major, new program)
• I-20/DS-2019 Extension  -- To request an extension in order to complete required coursework
• Notice of Future Intentions  -- To be completed by students who are departing Georgia State University for any reason
• Optional Practical Training (OPT) Form  and I-765 Form --Please print and complete both forms together.
• Applicant for Permanent Residency -- For students who have filed a Permanent Residency petition with USCIS
