If you wish to refer a student to University Career Services, please complete use the form below. 

We know it takes a team approach to assist students and we actively partner with faculty, advisors, and staff to support student success.

What to do?

Just complete the form below and then your request will be submitted directly to the Associate Director for Career Development, Karen Walker.

Once your student referral form has been received University Career Services will make 3 attempts to connect with the student to schedule a career counseling appointment.  Use of phone and e-mail contact will be used.

Our office will send a confirming e-mail to your attention when an appointment has been scheduled and attended.

By submitting this form the advisor confirms the below mentioned student has consented to this referral and is aware that University Career Services will attempt to schedule a career counseling appointment.
